Request For Termination Of Water Service

Welcome to Water Works District No. 3 of Rapides Parish, located in the Central Louisiana region.  It is our mission to provide you with safe, clean water, and to make the process of becoming a new customer as stress-free as possible.  To begin, please fill out the form below, attach a copy of your picture ID, and submit it to our office.

If you currently have service with Water Works District No. 3 and wish to transfer to a new address, please fill out the Transfer Form.

Customer must show proof of I.D. that they are the account holder.
If the account holder is deceased a death certificate or Power of Attorney must be shown by person requesting termination.
