Welcome to Water Works District No. 3 of Rapides Parish, located in the Central Louisiana region.  It is our mission to provide you with safe, clean water, and to make the process of becoming a new customer as stress-free as possible.  To begin, please fill out the form below, attach a copy of your picture ID, and submit it to our office.

AUTHORIZATION AGREEMENT FOR DIRECT PAYMENTS (ACH DEBITS)

I/we hereby authorize Water Works District No. 3 of Rapides Parish, hereinafter called COMPANY, to initiate debit entries to my/our account indicated below at the depository financial institution named below, hereinafter called DEPOSITORY, and to debit the same to such account. I/we acknowledge that the origination or ACH transactions to my/our account must comply with the provisions of U.S. law, and all NSF charges apply if funds are unavailable on the due date.

Checking Savings

This authorization is to remain in full force and effect until COMPANY has received written notification from me (or either of us) of its termination in such time and in such manner as to afford COMPANY and DEPOSITORY a reasonable opportunity to act on it, with a minimum time of one complete billing cycle.

NOTE: ALL WRITTEN DEBIT AUTHORIZATIONS MUST PROVIDE THAT THE RECEIVER MAY REVOKE THE AUTHORIZATION ONLY BY NOTIFYING THE ORIGINATOR IN THE MANNER SPECIFIED IN THE AUTHORIZATION.
